This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our commitment

Tributary Behavioral Health Services is required by federal law (HIPAA) and federal substance use confidentiality regulations (42 CFR Part 2) to maintain the privacy of your protected health information (PHI), provide you with this notice of our legal duties and privacy practices, and follow the terms of this notice.

How we use and disclose your information

We use and disclose your PHI for treatment, payment, and health care operations. For example, we may share information among Tributary clinicians coordinating your care, with your insurance plan to obtain payment, and for internal quality and audit activities.

Special protections for substance use treatment records

If you receive substance use treatment from Tributary, federal law (42 CFR Part 2) provides additional protections. We will not disclose information identifying you as receiving substance use treatment without your written authorization, except in narrowly defined circumstances (medical emergencies, qualified service organization agreements, audits, court orders, and reports of crimes on premises or against personnel).

Your rights

  • You have the right to inspect and obtain a copy of your records.
  • You have the right to request a correction to your records.
  • You have the right to request restrictions on how we use or disclose your information.
  • You have the right to request confidential communications.
  • You have the right to receive an accounting of certain disclosures we have made.
  • You have the right to receive a paper copy of this notice.
  • You have the right to file a complaint with us or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated.
