What started as a single counseling clinic in Lexington in 1989 has grown into a 12-site behavioral health system serving 18,000 people every year across three states.

Our Story

Named for the small streams that feed the Kentucky River.

Tributary was founded in 1989 in Lexington, Kentucky — established by a coalition of community physicians, a local Lutheran church, and the family of a Lexington high schooler whose suicide had exposed how few options existed for adolescent mental health care in central Kentucky.

The organization was named for the small streams that feed the Kentucky River — a reminder that meaningful care is built from many smaller acts of presence, listening, and showing up over time.

What began as one office on Nicholasville Road grew through 30+ years of community partnership, the integration of substance use treatment in 2003, residential programs in 2009, and most recently CCBHC certification in 2022. In 2023 Tributary expanded into West Virginia, and into Ohio in 2024, in response to the deepening behavioral health and opioid crisis across the Appalachian region.

Our History

From one clinic to twelve sites.

  1. 1989

    Founded as a single outpatient clinic in Lexington, Kentucky.

  2. 1994

    Expanded to Owensboro and Bowling Green.

  3. 2003

    Added substance use disorder treatment programs.

  4. 2009

    Opened first residential treatment facility in Lexington.

  5. 2014

    Expanded to Ashland; launched school-based services.

  6. 2019

    Received CARF accreditation across all sites; 30th anniversary.

  7. 2022

    CCBHC certification awarded for Kentucky operations.

  8. 2023

    Expanded to West Virginia: Charleston, Huntington, Morgantown, Beckley.

  9. 2024

    Expanded to Ohio: Athens, Marietta, Portsmouth, Chillicothe.

  10. 2025

    Launched primary care integration pilots in Lexington and Charleston.

  11. 2026

    Launched measurement-based care across all 12 sites.
